CNA Field -North Cook and Lake County
About The Position
The Field CNA is a paraprofessional member of the interdisciplinary group who works under the supervision of a registered nurse. The CNA performs services needed to meet patients’ personal needs and promote comfort, observes patients, and reports and documents observations and care provided. The role serves patients in their homes throughout North Cook and Lake Counties, including communities such as Evanston, Glencoe, Arlington Heights, Schaumburg, and Deer Park. The schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m. Reliable personal transportation and travel to patient homes are required.
Requirements
- High school diploma or GED.
- Meet the training requirements of Illinois.
- Current CNA certification.
- Valid driver’s license.
- Dependable, properly insured vehicle that complies with state and organizational standards.
- Reliable personal transportation and willingness to travel to patient homes throughout North Cook and Lake Counties.
Responsibilities
- Provide compassionate care, emotional support, and companionship to patients and families.
- Assist with personal care, mobility, and activities of daily living.
- Monitor, document, and report patient condition and changes to the registered nurse.
- Ensure patient safety and proper use of equipment.
- Maintain a clean, safe, and comfortable home environment.
- Follow care plans and policies, and participate in training and team activities.
- Travel to patient homes throughout North Cook and Lake Counties.
