About The Position

The Field Chaplain will be responsible for the spiritual care assessment and participate in the development of the interdisciplinary plan of care for the terminally ill hospice patients and their families, as assigned. The Chaplain will ensure the best spiritual outcomes as the patient desires or requests. Halifax Health serves Volusia and Flagler counties, providing a continuum of health care services through a network of organizations including a tertiary hospital, two community hospitals, urgent care clinics, psychiatric services, a cancer treatment center with five outreach locations, the area’s largest hospice, a center for inpatient rehabilitation, outpatient rehabilitation clinics, primary care walk-in clinics, a clinic specializing in women’s health, a pediatric care community clinic, five pediatric medical practices, a home health care agency and an exclusive provider organization. Halifax Health offers the area’s only Level II Trauma Center, Thrombectomy-Capable Stroke Center (TSC), Center for Transplant Services, Pediatric Intensive Care Unit, Child and Adolescent Behavioral Services, complete Neurosurgical Services, OB Emergency Department and Level III Neonatal Intensive Care Unit that cares for babies born earlier than 28 weeks.

Requirements

  • Masters of Divinity, M.Div., required
  • Member of the College of Chaplains
  • One basic unit of completed CPE required
  • Sanctioned by a recognized religious organization
  • Excellent time management, organizational skills, self-awareness and boundaries which are modeled for others
  • Valid driver's license with a good driving record; personal car in good working order

Nice To Haves

  • Previous experience in Hospice or a health care setting preferred

Responsibilities

  • Provide and document spiritual care assessments
  • Provide spiritual support and services to the patient/caregiver
  • Facilitate contact with community spiritual support
  • Assess bereavement needs
  • Provide crisis intervention
  • Serve as a resource to the interdisciplinary team
  • Participates in education of the community regarding Hospice care
  • Provides direction and supervision to assigned Spiritual Care Volunteers
  • On-call rotation
