The Field Case Manager - GTA (Toronto) role is responsible for customizing program elements to meet the needs of assigned prescribing physicians, reviewing patient status, and assisting physicians with patient enrollment and accessing public or private coverage. This role involves identifying obstacles to coverage, developing strong relationships with physicians and their staff, providing training on program initiatives, and acting as a liaison to the Program Manager. The position also involves patient support, including welcoming patients, providing reimbursement information, coordinating coverage, confirming outcomes, assisting with financial aid, and coordinating therapy. Additionally, the role includes mentorship and peer support, interfacing with the manufacturer, and completing various reports. The Field Case Manager may also be assigned other duties and may be required to be on standby or on call.
