About The Position

The Field Care Manager Nurse 2 assesses and evaluates members' needs to achieve and maintain wellness. The nurse guides members/families toward appropriate resources and facilitates interaction with them for the care and wellbeing of members. This role involves varied work assignments, frequently requiring interpretation and independent determination of appropriate courses of action. The Field Care Manager Nurse 2 employs a variety of strategies and techniques to manage a member's physical, environmental, and psycho-social health issues, resolving barriers that hinder care. The nurse ensures patients are progressing towards desired outcomes by managing patient care through assessments and evaluations and may create member care plans. The position requires understanding department, segment, and organizational strategy and operating goals, including their linkages to related areas. Regarding own work methods, the nurse occasionally receives guidance where needed and follows established guidelines/procedures. The role is located at CenterWell Jacinto City, 1910 John Ralston Rd., Houston, TX 77013 and reports to the Associate Operations Director, Clinic/Center Administration.

Requirements

  • Registered Nurse with 2 years of experience of in-home case/care management
  • Experience working with the adult population
  • Knowledge of community health and social service agencies and additional community resources
  • Ability to travel to member's residence within 30 to 40 miles
  • Use a variety of electronic information applications/software programs including electronic medical records
  • Excellent keyboard and web navigation skills
  • Ability to work a full-time (40 hours minimum) flexible work schedule
  • Participate in Humana's driver safety program
  • Valid state driver's license
  • Maintain personal vehicle liability insurance (minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher)
  • Must have a separate room with a locked door that can be used as a home office to ensure privacy while you work
  • Must have accessibility to high-speed DSL or Cable modem for a home office (Satellite internet service is NOT allowed for this role); and recommended speed for performance from Humana at Home systems if 5Mx1M
  • Requires screening for Tuberculosis (TB)
  • Valid driver's license, car insurance, and access to an automobile
  • Associates working in the State of Florida will need ACHA Level II Background clearance
  • Must be passionate about contributing to an organization focused on improving consumer experiences
  • Self-provided internet service must meet a minimum download speed of 25 Mbps and an upload speed of 10 Mbps (wireless, wired cable or DSL connection suggested; Satellite, cellular and microwave connection leadership approves only if used)
  • Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information

  • Previous managed care experience
  • Bilingual — English, Spanish
  • Certification in Case Management
  • Motivational Interviewing Certification or knowledge
