Requirements

Current unrestricted Texas RN license
2+ years of experience in a community health or healthcare setting
Intermediate level of proficiency with Microsoft Office applications
Reliable transportation, a valid driver’s license, and ability to travel within assigned territory
High-speed internet at residence

Nice To Haves

Experience with Medicaid, LTSS, or long-term care services
Knowledge of HCBS and integrated care principles
Knowledge of the principles of most integrated settings, including federal and State requirements like the federal home and community-based settings regulations
Bilingual in English and Spanish

Responsibilities

Assess, plan, and implement person-centered strategies
Connect members with financial, psychosocial, community, and state resources
Coordinate care across the continuum and serve as a key point of contact
Advocate for members and families to ensure their needs and preferences are heard and supported
Collaborate with providers, members, caregivers, and community partners to improve health outcomes
Travel 50% of the time within the Bexar County, TX area and surrounding areas
