About The Position

The Salvation Army Syracuse Area Services is excited to announce the expansion of our Functional Family Therapy–Child Welfare (FFT-CW) Program. We are seeking a dedicated and career-minded Level II Case Planner to join our team in a full-time, 35-hour-per-week position with an outstanding benefits package. In this position, you will participate in specialized FFT-CW training and provide evidence-based therapeutic services to 10–12 families. You will use the FFT-CW family systems model to assess needs, build strengths, and support positive family outcomes. You will work closely with children, caregivers, and support networks to improve family functioning and stability. You will identify and address risk factors while strengthening protective factors to enhance child safety and well-being. You will develop and implement individualized service plans tailored to each family's unique needs. You will collaborate with community providers and child welfare partners to coordinate services and resources. You will support family preservation efforts and help prevent the out-of-home placement of children. Requirements

  • Master’s Degree or working towards a Master's Degree is preferred.
  • NYS Driver License and access to private transportation

Responsibilities

  • Conduct comprehensive intakes, assessments, and ongoing evaluations to identify client and family strengths, needs, and service goals.
  • Provide FFT-CW services through regular client engagement, family therapy sessions, and required contacts to support family stability and positive youth outcomes.
  • Develop and implement individualized service plans in collaboration with families, Department of Social Services (DSS), and community providers, using FFT-CW strategies to promote behavioral change and family preservation.
  • Participate in FFT-CW training, supervision, consultations, and case conferences while maintaining clinical assessments, outcome monitoring, and program documentation.
  • Deliver case management, counseling, crisis intervention, advocacy, referrals, and service coordination to address client and family needs.
  • Complete timely and accurate case records, reports, assessments, narratives, statistical data, and recommendations in accordance with program standards.
  • Maintain strong partnerships with DSS and community agencies, provide case recommendations, and demonstrate cultural competence in all aspects of service delivery.
