About The Position

Catholic Charities Brooklyn and Queens (CCBQ) is seeking a Fee for Service Licensed Social Worker (LMSW) to join their team. CCBQ has a 125-year history of providing social services and offers over 160 programs for various populations, including those with mental illness. Their clinics are community-based, offering a professional and welcoming environment. Services provided include psychotherapeutic and psychopharmacological therapy, individual, group, and family therapy, crisis intervention, medication management, and case management. The approach emphasizes wellness, recovery, trauma-informed care, and physical-behavioral health integration.

Requirements

  • Master’s Degree in Social Work from a nationally accredited institution.
  • Valid NYS LMSW required.
  • Obtain NPI immediately upon employment.
  • Must obtain and maintain all licensure/certifications for which employee is educationally and professionally eligible in NY State.
  • Excellent Time Management, organizational and computer skills.
  • Ability to evaluate and intervene in crisis situations.
  • Flexibility.
  • Excellent oral & written communications skills.

Nice To Haves

  • Bilingual Spanish or Creole speaking preferred.

Responsibilities

  • Assess client needs holistically and connect clients with internal and external services.
  • Provide direct service to clients in outpatient mental health programs under the direction of a Clinical Supervisor.
  • Function as part of a treatment team.
  • Maintain mandated levels of service.
  • Participate in Agency meetings as required.
  • Comply fully with Agency policies and procedures.
  • Perform initial interviews, crisis intervention, and ongoing therapy with individuals, groups, and families.
  • Adhere to all standards of contract compliance, audit readiness, Agency policies and procedures, and regulatory requirements.
  • Write progress notes, treatment plans, intake assessments, correspondence, termination summaries, and encounter forms.
  • Make referrals to appropriate internal and external services.
  • Respond to and follow up on referrals in a timely and professional manner.
  • Conduct individual, family, marital, and group therapy.
  • Monitor and coordinate psychiatrist psychopharmacology appointments.
  • Coordinate physical, medical, and psychological needs for clients.
  • Complete and submit all documentation required for Agency reimbursement within 24 hours of session.
  • Maintain all applicable standards of audit readiness.
  • Attend and actively participate in supervision.
  • Participate in Agency response to serious incidents as needed.
  • Participate in meetings with supervisor and other staff for training, planning, and evaluation.
