Rock Valley Community Programs provides transitional and supportive housing, addiction and mental health treatment services to adults in the Rock County area. We provide these services to homeless male veterans, clients connected to the criminal justice system, and Rock County individuals needing assistance with addiction and/or mental health treatment. The Federal Case Manager provides case management, re-entry planning, and care coordination services for residents participating in a federal residential re-entry program. Working under the supervision of the Federal Program Director, this position supports continuity of care and successful community reintegration through individualized programming and service coordination. The Federal Case Manager maintains resident caseloads, conducts resident meetings, coordinates collateral contacts with contractors and community resources, documents resident activities, and monitors progress toward re-entry goals. This position also attends required meetings and trainings, completes reporting requirements, and ensures compliance with all Federal Bureau of Prisons (BOP) Statement of Work (SOW) requirements and Residential Services Department Policies and Procedures. The Federal Case Manager must maintain positive working relationships with contractors and community partners while consistently representing the organization in a professional manner.
