About The Position

Rock Valley Community Programs provides transitional and supportive housing, addiction and mental health treatment services to adults in the Rock County area. We provide these services to homeless male veterans, clients connected to the criminal justice system, and Rock County individuals needing assistance with addiction and/or mental health treatment. The Federal Case Manager provides case management, re-entry planning, and care coordination services for residents participating in a federal residential re-entry program. Working under the supervision of the Federal Program Director, this position supports continuity of care and successful community reintegration through individualized programming and service coordination. The Federal Case Manager maintains resident caseloads, conducts resident meetings, coordinates collateral contacts with contractors and community resources, documents resident activities, and monitors progress toward re-entry goals. This position also attends required meetings and trainings, completes reporting requirements, and ensures compliance with all Federal Bureau of Prisons (BOP) Statement of Work (SOW) requirements and Residential Services Department Policies and Procedures. The Federal Case Manager must maintain positive working relationships with contractors and community partners while consistently representing the organization in a professional manner.

Requirements

Degree and/or equivalent experience in Human Services, Criminal Justice, or Counseling.
Strong written and verbal communication skills.
Ability to work collaboratively within a multidisciplinary environment.
Ability to manage high volume client interactions in a group setting.
Proficiency in Microsoft Office.
Must be able to sit, stand and walk for extended periods of time
Occasionally lift/move up to 25 pounds.
Valid driver’s license and clean driving record
Must complete TB test, and drug screening (on-site).
Completion of CPR, First Aid, CBRF Fire Safety, and Choking training within 90 days
Employment is contingent upon passing a state and federal background check.

Responsibilities

Develop and maintain individualized re-entry plans that address housing, employment, financial stability, health needs, education, and successful community reintegration.
Conduct regular individual sessions to monitor progress, address barriers, reinforce accountability, and support release planning.
Assist residents with securing housing, employment, educational opportunities, and community resources.
Monitor resident behavior, program participation, and compliance with RVCP and Federal Bureau of Prisons (FBOP) requirements.
Review and approve resident passes, monitor home confinement and GPS compliance, and ensure required registrations and reporting obligations are met.
Complete employment verification and ongoing employer contacts as required by the FBOP Statement of Work (SOW).
Coordinate services with USPOs, Therapists, Employment Placement Specialists, community providers, and other stakeholders to ensure continuity of care.
Provide referrals and facilitate access to services that support resident success and re-entry goals.
Maintain professional relationships with residents, contractors, community partners, and agency staff.
Provide assistance to program support staff as needed to include: conducting observed UAS, room searches, headcounts, facility walk throughs.
Complete and maintain all required documentation, including intake paperwork, Individualized Program Plans, progress reports, case notes, incident reports, home confinement paperwork, exit plans, and discharge summaries.
Ensure all reporting is completed accurately and within required timelines to meet FBOP SOW and agency standards.
Audit resident files for compliance, present resident updates at Program Review Team (PRT) meetings, attend required meetings and trainings, and assist with facility operations as needed, including facility searches and computer lab monitoring.
