About The Position

The Family Therapist is responsible for the psychosocial assessment, discharge planning, and individual/group/family therapy on the inpatient unit. The Family Therapist is a vital member of the interdisciplinary treatment team, attending all treatment team meetings and actively participating. The Family Therapist coordinates communication between the treatment team and the family for the purpose of assessing patient needs and planning for discharge/necessary follow-up care. The Family Therapist provides support to the family and patient and acts as a liaison between community agencies to ensure seamless care for the patient during the inpatient stay and follow-up care post discharge.

Requirements

  • State of Ohio, Licensed Social Worker (LSW) or Licensed Professional Counselor (LPC) required.
  • Master’s Degree in Social Work or Counseling required.
  • Competence in computer skills (Word, Excel, Internet and e-mail) required.
  • Excellent written and verbal communication skills required.
  • Must possess a high level of integrity in dealing with confidential data.
  • Flexibility in daily assignment as needed.
  • Able to communicate with patients and families both in writing and oral communication.

Nice To Haves

  • Licensed Independent Social Worker (LISW) or Licensed Professional Clinical Counselor (LPCC) preferred.
  • Previous social work experience in a health care setting preferred.

Responsibilities

  • Perform psychosocial assessment, discharge planning, and individual/group/family therapy on the inpatient unit.

  • Communicate with team through accurate documentation.
  • Participate in treatment team meetings utilizing open communication with team members.
  • Adhere to principles of infection control as defined by risk assessment of the job.
