About The Position

Our programs span early childhood education, youth development, healthy aging, mental health and wellness, English classes, community centers, and the arts. The Prevention Program, funded by the Administration for Children's Services (ACS), offers comprehensive home and community-based preventive services. This program integrates Mobility Mentoring, an evidence-based model focused on growth mindset and high expectations for children and families. The Family Team Conference (FTC) Facilitator is a crucial team member, ensuring family voice and choice are central to the work and planning, and that Mobility Mentoring principles are integrated. The Facilitator supports family stability and growth by connecting them with resources and services to achieve their goals. The role involves coordinating and conducting FTCs using the ACS-approved six-stage model, ensuring family perspectives are highlighted in discussions about prevention services and progress. A key aspect of the role is supporting the integration of Mobility Mentoring into family plans and collaborating with the Prevention Program team to achieve goals.

Requirements

• Masters degree in Social Work or two (2) years casework experience and one (1) year group work experience
• At least two years of work experience required in a child welfare-related setting facilitating groups and/or team meetings
• Must have excellent group facilitation and communication skills and work well on a team
• Organized, able to manage multiple responsibilities concurrently including during a crisis.
• Proficiency in Microsoft Office (Outlook, Excel, and Word) required
• Must have a commitment to work from a strength based, youth and family development, and trauma-informed perspective that is grounded in an understanding of the impact of poverty, oppression and structural racism.
• Dedicated to being a team player and active contributor to achieve the vision and mission of the program and agency.

Nice To Haves

• Minimum 1 year Supervisory experience is a plus
• Familiarity with Connections and PROMIS databases a plus
• Bilingual in English/Spanish is a plus

Responsibilities

• Facilitate FTCs for families and staff to review services, assess progress together, and help develop a plan of action to either ensure further progress towards positive child and family outcomes or end services as appropriate
• Utilize the six-stage model for FTCs and encourage and support the use of Mobility Mentoring in interactions and planning with families and staff
• Maintain an active and flexible FTC schedule that meets the needs of families and compliance guidelines of ACS, including responding to crisis intervention, changing family needs and/or newly required FTCs
• Serve as an active member of the interdisciplinary team including attending case conferences, reviewing each family's case record in preparation for the FTC and maintaining communication with team members during the life of the case to support the integration of Mobility Mentoring and positive family outcomes
• Conduct visits to participant homes in order to hold conferences when needed
• Submit accurate and timely documentation summarizing each FTC as required by ACS in PROMIS and Connections databases
• Conduct follow-up reviews to ensure that tasks are occurring as planned
• Facilitate ongoing and/or short-term participant groups or workshops to address themes impacting families in the program
• Attend and participate in all meetings, case consultations, supervision and related agency and external training as appropriate
• Perform other duties as assigned by the supervisor
