A key responsibility is working with families to develop family support plans and conducting interviews to gather general household data. The role also coordinates necessary transportation, maintains client files, participates in trainings, and makes referrals for services. Additionally, it involves maintaining liaison with community agencies to track service outcomes and providing phone and client contact support.
