About The Position
The FFT-FC Family Support Specialist works in conjunction with the FFT FT Clinician providing in home support services using an evidenced based model. This position is responsible for case management, support, being the bridge to other service providers, conducts assessments, implements treatment plans and other paperwork to support the treatment model.
Requirements
- BA in Social Work, psychology, or another related human service degree.
- Bilingual (Spanish).
Responsibilities
- Provide in-home support services using an evidenced based model.
- Perform case management.
- Provide support to families.
- Act as a liaison to other service providers.
- Conduct assessments.
- Implement treatment plans.
- Complete necessary paperwork to support the treatment model.
