We integrate medical, behavioral, and social care, prioritizing prevention and recovery. We are looking for a mission-aligned Family Support Specialist who will leverage lived caregiver experience to enhance our care model. This role involves partnering with caregivers of youth facing various challenges, offering support, building trust, and guiding them through complex systems to achieve lasting wellness. The ideal candidate will use their personal experience to build authentic relationships and provide hope to families navigating health, behavioral, and social difficulties.

Requirements

  • Lived experience as a caregiver of a child or youth with emotional, behavioral, mental health, substance use, or developmental challenges, including navigating child-serving systems on their behalf.
  • 18 years or older with a high school diploma or GED.
  • At least 12 months (within the past 10 years) of paid or volunteer experience supporting families or navigating child-serving systems.
  • Willingness to work under the supervision of a licensed mental health professional.
  • Comfort with technology, including telehealth, smartphone apps (Holon Vibe app), and basic digital documentation.
  • Tennessee residency.
  • Reliable transportation.
  • Ability to make occasional community or field visits.
  • Ability to pass background checks required for working with minors and families.
  • Strong boundaries, cultural humility, and a deep commitment to family voice and choice.

Nice To Haves

  • Tennessee Certified Family Support Specialist (CFSS), or eligibility and willingness to obtain within 12 months of hire.
  • National Federation of Families Certified Family Peer Specialist (CFPS) credential.
  • Experience with juvenile justice, child welfare, truancy/school, or youth behavioral-health systems (especially for Memphis/Shelby County youth work).
  • Familiarity with engagement or contingency-management models.
  • Bilingual (Spanish).

Responsibilities

  • Partner with caregivers as a peer, using lived experience to build trust, offer hope, and be a reliable point of contact.
  • Drive family engagement in the program by assisting families with program initiation, appointment adherence, and utilization of the Holon Vibe app for daily check-ins, caregiver lessons, and engagement rewards.
  • Reinforce the caregiver curriculum by modeling and supporting communication and problem-solving skills taught in the program.
  • Navigate and advocate across systems, including schools, courts, child welfare, community resources, and existing behavioral-health providers, while teaching caregivers self-advocacy skills.
  • Build the family's support network and help caregivers identify natural supports and set family goals aligned with the care plan.
  • Coordinate with the Holon care team, including keeping primary care providers and case managers informed, flagging concerns, and ensuring the family remains central to the care plan.
  • Document engagement (non-clinical) by recording participation and progress in the platform, adhering to guardian consent, youth privacy, and Holon's compliance rules.
