Requirements

Associates degree in health and human service-related field or education
2 years of experience
Valid driver's license
Auto insurance
Good driving record
Ability to pass an extensive background check including checks of the Dependent Adult and Child Abuse registries, Sex Offender registry and a criminal history check.

Responsibilities

Offer home visits to expectant parents and caregivers of small children, providing parenting education, support, and advocacy.
Link parents to resources.
Promote optimal child health and development.
Problem solve with parents.
Evaluate family needs and abilities on an ongoing basis.
Assist families in designing individualized support plans.
Empower families overtime to achieve their chosen goals.
Primarily meet with clients in the home and maintain contact via telephone in Black Hawk County.
