The Family Support Services Supervisor is a full-time, 40-hour per week position responsible for coordinating all aspects of patient social services, including social work, chaplaincy, and bereavement, as well as volunteer coordination and community outreach. This role involves establishing, implementing, and evaluating goals and objectives to meet and promote quality standards and contribute to the PVHPC mission and philosophy. The supervisor will oversee the Family Support Services Department, which includes the Volunteer Coordinator, Bereavement Coordinator, Chaplains, Social Workers, and Integrative Therapists. Key responsibilities include developing and implementing departmental objectives, policies, and procedures, ensuring services are delivered in accordance with the plan of care and regulatory standards, and coordinating care with other clinical service providers. The position also involves recruiting, orienting, evaluating, and managing the Family Support Services Team, and assuring proper documentation in clinical records. The supervisor will contribute to strategic planning, maintain knowledge of community resources, and assist patients and families in accessing them. They will also plan and implement educational programs for the team, attend IDT meetings, participate in quality improvement processes, monitor caseloads, and develop community outreach services. Administrator on-call responsibilities may be required, and the supervisor will maintain a manageable caseload.
