About The Position

The Family Support Provider (FSP) - Team Leader is responsible for assisting with the delivery of Family Support Services for clients. This role involves identifying potential candidates for recruitment, coordinating family support services throughout CPRC programming and other agency departments, and participating in activities associated with stigma reduction, consumer engagement, and follow-up. The position requires travel to Fredericktown, Perryville, Ste Genevieve, and Marble Hill locations. This is a full-time, non-exempt (hourly) position.

Requirements

  • High School Diploma / GED / HSE (at minimum)
  • Must be a family member with a child who has been, or currently has, a behavioral or emotional disorder and has received mental health services.
  • Must successfully complete the FSP training as approved and provided by MO DMH
  • Must meet appropriate background checks from DSS, DMH, and Law Enforcement Agencies
  • Must be a Missouri Resident for at least two years.
  • Travel in own vehicle is required
  • Must hold or obtain by start date, MO Class E Driver's License (Class D for IL Residents), and proof of continuous auto insurance.
  • Must also meet CCC's Motor Vehicle Record standards policy and have reliable transportation.

Responsibilities

  • Provide parents and caregivers with essential information about their child/youth's needs.
  • Explore treatment options collaboratively, ensuring informed decision-making.
  • Guide parents/caregivers through the planning process.
  • Emphasize the importance of their voice in developing and implementing individualized treatment plans.
  • Empower parents/caregivers to advocate effectively during planning meetings.
  • Ensure their perspectives are heard and valued.
  • Work closely with families to highlight the significance of individualized planning.
  • Leverage their inherent strengths to achieve positive outcomes.
  • Educate families on the roles of various providers.
  • Promote a cohesive "team" approach to holistic care.
  • Discuss the benefits of natural family and community supports.
  • Encourage utilization of existing networks.
  • Introduce effective problem-solving methods.
  • Develop strategies to address challenges.
  • Assist parents/caregivers of Transition Age Youth.
  • Facilitate the shift from decision-maker to supportive role as youth become more independent.
  • Link families to vital community resources.
  • Strengthen community bonds.
  • Foster active engagement with families.
  • Reconnect with those who may have disengaged.
  • Provide education on services available through CCC (Community Care Center).
  • Engage with the community to raise awareness.
  • Educate about available resources.
  • Collaborate as a valued member of the clinical team.
  • Contribute insights from the client's perspective.
  • Adhere to clinical policies, DMH (Department of Mental Health) guidelines, CARF (Commission on Accreditation of Rehabilitation Facilities) standards, and federal/state program requirements.
  • Responsible for safe transportation of clients using agency or personal vehicles as needed.
