About The Position

NAMI San Diego & Imperial Counties (National Alliance on Mental Illness) is a nonprofit organization that provides free education, support services, and advocacy to improve the quality of life of everyone affected by mental health concerns. This position is based in Imperial County and is hybrid, involving work from a home office, the NAMI office, and in the community. The Family Support Navigator will support the Coordinated Family Supports (CFS) program, assisting adults who are clients of the San Diego Regional Center in Imperial County, and their families, in connecting to services. The role involves case management, navigating and accessing services, identifying and overcoming barriers, planning, education, and other related services to help individuals remain successfully in their family's home.

Requirements

  • Bilingual (English/Spanish).
  • Associate-level degree in a human services field of study or 3 years of experience in the developmental disabilities service delivery system.
  • Ability to communicate in the primary language of the client and their family.
  • Knowledge of the Regional Center system.
  • Bachelor's degree in a human services field of study (such as psychology, social work, child development, etc.); OR an Associate's degree in a human field of study and at least 3 years of experience supporting individuals living with intellectual/developmental disabilities (IDD).
  • At least one year of experience supporting individuals living with intellectual/developmental disabilities.
  • In-depth knowledge of Imperial County's services, resources, and culture.
  • Exceptional interpersonal skills, positivity, attention to detail, flexibility, and enjoyment working in an ever-changing environment.
  • Excellent planning, organizing, problem-solving, time management, and coordinating skills.
  • Able to work with minimal direction and supervision.
  • Demonstrated ability to maintain confidentiality.
  • Strong positive viewpoint towards individuals with developmental disabilities, mental health challenges, and a history of substance use, and their families.
  • Natural ability to build and maintain relationships and find common ground with diverse stakeholders.
  • Ability to get along and work effectively with others.
  • Able to communicate effectively, verbally and in writing.
  • Regular, predictable attendance is required.
  • Intermediate to advanced knowledge of computer software (Microsoft Office, Word, Excel), email, and web navigation.
  • Must be willing and able to drive your personal car, possess valid California Driver's License and comprehensive automobile insurance as required by law.
  • Must have a satisfactory driving record.
  • Applicants will be required to undergo a background check following a contingent offer of employment and must receive satisfactory results prior to a final offer of employment.

Nice To Haves

  • Personal lived experience or with a family member or loved one with developmental disabilities and/or behavioral health challenges, preferred.
  • Completion of Medi-Cal Peer Support Specialist Training is preferred.

Responsibilities

  • Maintain and manage an active caseload.
  • Complete assessments to determine client's needs and develop written support plans.
  • Maintain contact and meet regularly with clients.
  • Connect individuals and their families with needed resources, providing guidance, identifying and addressing barriers, supporting benefit applications, providing education, and system navigation.
  • Complete follow-up assessments to determine client progress and update plans.
  • Document services, intakes, and visits within three business days.
  • Attend and participate in individual supervision.
  • Communicate and collaborate with staff from San Diego Regional Center, clients' family members, and other providers.
  • Participate in meetings with the CFS team, San Diego Regional Center, other providers, and NAMI San Diego & Imperial Counties.
  • Build and maintain relationships, conduct outreach activities with the community and providers/staff of the San Diego Regional Center and other providers.
  • Support the Lead Family Supports Navigator with report completion and invoicing.
  • Provide culturally and linguistically appropriate services.
  • Work collaboratively in a teamwork style.
  • Other duties as assigned.
