About The Position

As a Family Support Coordinator you’ll play a vital role in advancing our mission. In this position, u nder the general direction of the Manager, Family Support Services and the Manager On Call (MOC), you’ll be responsible for obtai ning authorization for donation, provides organ-donation-informed emotional support and guidance to family members of potential organ donors by providing assistance with end- of- life decisions by providing timely assessment and support where applicable, facilitating the approach for organ and tissue donors, and supporting the family with the transition to the organ recovery phase of the process. The FSC is required to make independent decisions when approaching families based on training and experience. Collaborates with hospital staff as well as other LNH staff to determine the appropriate time to approach the family to obtain authorization and perform Donor Risk assessment form. Your work will have purpose every single day, contributing directly to life-changing outcomes.

Requirements

  • Bachelor’s Degree – Social Work, End of Life/Bereavement Care, Pastoral Care, Crisis intervention or related field
  • Valid Driver’s License
  • Three (3) years’ experience with Social Work, End of Life/Bereavement Care, Pastoral Care, Healthcare, or related Field
  • INTERPERSONAL SKILLS: Ability to communicate effectively with colleagues, hospital staff, and families in crisis, as well as community leaders .
  • COMMUNICATION SKILLS: Ability to effectively communicate verbally and in writing, building relations with all professionals at different levels within the organization.
  • CLINICAL KNOWLEDGE : Able to learn medical terminology and common medical conditions leading to brain death. Able to discuss medical conditions with medical personnel.
  • TIME MANGEMENT : Able to prioritize multiple, competing priorities and manage time/workload. Demonstrated ability to effectively prioritize and juggle multiple time sensitive projects, multi-task, identify project interdependencies and potential risks/pitfalls.
  • ATTENTION TO DETAIL : Able to perform tasks thoroughly and with care; checks work to ensure a high degree of accuracy/completeness and early/on-time delivery.
  • PROFICIENT IN MS OFFICE: PowerPoint, Excel, Word, Outlook, Microsoft Suite
  • PSYCHOLOGICAL SKILLS: Ability to work under extreme pressure and high emotional situations. The ability to cope with issues pertaining to death.
  • COLLABORATIVE: Ability to build internal and external relationships with staff, hospital, families in crisis and other leaders.

Nice To Haves

  • Master’s Degree – Social Work, End of Life/Bereavement Care, Pastoral Care, Crisis intervention or related field
  • Five (5) years prior experience in an OPO or Transplant Center or experience with care of donor family

Responsibilities

  • Family Engagement & Advocacy: Obtains authorization for donation. Works empathetically, compassionately, and professionally with families in crisis to advocate for donation and transplantation. Provides timely support to families that are in need and have the possibility of becoming a donor. Focuses on Donation after Cardiac Death and non-registered donor potential donors. Communicates complicated clinical topics.
  • Trauma-Informed Care & Management : Develops effective professional and personal relationships quickly and possesses superior communication skills. Demonstrates the ability to engage in emotionally charged situations and display their experience in crisis assessment, intervention, and counseling.
  • Donation Advocacy with Hospital Partners : Works within hospital to increase physician and other hospital personnel openness and responsiveness to potential organ and tissue donation by developing and maintaining effective relationships with key physicians and other personnel.
  • Authorization Process Management : Conducts the Donor risk assessment interview with the family or next of kin and documents accurately in compliance with quality systems and corporate programs.
