About The Position

Under limited supervision, establishes and oversees the referral process and service placements for family support, community, and residential based service. This position works closely with the Director of IDD, Residential Manager and IDD Day Services Manager and service leadership to recruit, connect and establish clients into services.

Requirements

Bachelor's degree from an accredited college or university in a related field AND Five years of experience managing professional level staff OR Nine years of experience AND Five years of management experience at the level equivalent to area of assignment
An understanding of the developmental disability population, diagnosis and general knowledge of assessment tools used in determining cognitive functioning.

Master’s degree in special education or related field.
Work experience and/or knowledge of resources in communities for children and disabled adults.
Work experience and/or knowledge of Individual Education Plans, 504 Plans and IDEA laws.
Work experience and/or knowledge of DBHDD policies and procedures of NOW/COMP Waiver services requirements and definitions.

Responsibilities

Participates as a member of the interdisciplinary to determine eligibility for individuals seeking applications for developmental disability services.
Reviews assessments, IEP’s, individual support plans, writes assessments, updates, technical assistance and consultation to determine service delivery options.
Conducts interviews with consumers, family members, legal guardians, caregivers and primary care physicians as needed.
Makes services recommendations, writes IFSP’s and action plans to streamline intake to services.
Follows up on actions plans with programs and designated staff to ensure follow up and time lines are established and met to provide services.
Networks and coordinates with community partners to provide education and resources for IDD services and linkage.
Provides person centered planning and self-determination for individuals entering services.
Coordinate’s information sharing between IDD Program Leadership, Core agency programs and all other invested parties.
Anticipates problems and responds proactively; troubleshoot concerns.
Works closely and collaboratively with the billing department, human resources, Care Management to increase program performance and quality; provides technical assistance when needed.
Development of the Individual Family Support Plan (IFSP) to support the individual and family in multiple facets of life, including home, school and community and assistance in identifying both their long and short-term goals.
Create and track annual budget of funds based on IFSP plan development.
Maintains Individual/Family file to include annual and bi-annual IFSP Reviews, data entry information into Connects systems.
Monitor services and goods the family is receiving under the IFSP.
Ensures ongoing adherence and meeting of requirements to all licensing bodies, DBHDD, DCH, Medicaid rules and regulations and state and regional policy.
Seeks resources for families and considers concerns, strengths and priorities for children and individuals of different ages and with various types of disabilities and conditions.
Work with others in public and private service systems to negotiate a plan that meets the needs of the family.
Relate to families and individuals with diverse ethnic, economic, and cultural backgrounds and circumstances.
Serve as a support and guided counsel to families as needed to include listening to problems and concerns as well as successes and gains.
Represents the agency and program in an informed and courteous manner with all external contacts and community linkages.
Demonstrates a diverse knowledge of rules, regulations that govern the agency.
Maintains professional behavior when completing tasks and interacting with others as a team member.
Communicates effectively and collaboratively with others to obtain information; ensures response within designated timeframe.
Promotes customer satisfaction and assists region with resolving grievances, complaints and inquiries.
Participates in internal and external educational programs and professional meetings as available for continuing professional education.
Participates in agency meetings as necessary to ensure continuity.
