About The Position

The Family Practice Physician provides comprehensive, patient-centered primary care services to individuals and families across the lifespan within a Federally Qualified Health Center Look-Alike (FQHC Look-Alike) setting. This role is dedicated to serving a diverse and often underserved population, with a strong emphasis on preventive care, chronic disease management, and whole-person health. The Physician works collaboratively within an interdisciplinary care team to deliver high-quality, culturally responsive care aligned with FQHC Look-Alike mission and values.

Requirements

  • MD or DO from an accredited medical school
  • Completion of an accredited Family Medicine residency program
  • Board certified or board eligible in Family Medicine
  • Active and unrestricted medical license in WI
  • DEA certification
  • Basic Life Support (BLS) certification

Nice To Haves

  • Prior experience working in an FQHC, community health center, or underserved setting
  • Bilingual skills
  • Experience with integrated care models (behavioral health, dental coordination)
  • Familiarity with value-based care and quality metrics

Responsibilities

  • Provide full-spectrum primary care services, including diagnosis, treatment, and management of acute and chronic conditions for patients of all genders and ages
  • Deliver preventive care services, including annual exams, immunizations, screenings, and health education
  • Manage chronic diseases such as diabetes, hypertension, and asthma using evidence-based guidelines
  • Perform routine in-office procedures
  • Provide women’s health services, including contraceptive counseling
  • Coordinate care with behavioral health, dental, and specialty providers as part of an integrated care model
  • Utilize electronic health records (EHR) to document patient encounters accurately and timely
  • Participate in care team huddles, case conferences, and quality improvement initiatives
  • Support population health efforts, including outreach and care gap closure
  • Provide culturally competent, trauma-informed care to a diverse patient population
  • Participate in after-hours call rotation as needed
  • Demonstrate commitment to serving underserved and vulnerable populations
  • Learn, understand, and support FQHC Look-Alike compliance requirements, including UDS reporting and HRSA guidelines
  • Contribute to achieving quality metrics and value-based care goals
  • Maintain productivity expectations while prioritizing quality and patient-centered care
  • Ensuring continuity of care
