About The Position

Comprehensive Community Action, Family Health Services, a Joint Commission Accredited, NCQA III Federally Qualified Community Health Center, is seeking a full-time Family Nurse Practitioner or Physician Assistant for a growing, multi-service, multi-site Family Medicine Practice. The Patient Centered Medical Home practice integrates oral and behavioral health care, along with many family-centered case management programs. Providers are assigned to a team consisting of a nurse, medical assistant, social worker, patient navigator, and nurse care manager. This patient-centered medical home approach supports the patient through all aspects of their care. The practice utilizes the NextGen electronic health record system, integrated across all disciplines. Opportunities are available for those with special population interests. The practice will sponsor J1 Visa applications with a multi-year contract. Experience in and a desire to work with underserved communities, increasing access to preventive and primary healthcare, is essential. The Health Center has four locations and offers a comprehensive approach to managing healthcare and individual/community social needs.

Requirements

  • Licensed in Rhode Island (or eligible)
  • Community oriented
  • Strong clinical background in Family Medicine
  • Strong communication skills
  • Strong commitment to community practice
  • Experience in and desire to work with underserved communities

Responsibilities

  • Supervision and oversight of clinical support teams
