Requirements

  • Completion of an accredited Medical School and Family Medicine Residency (for Physician).
  • Current California Medical License (MD or DO) (for Physician).
  • Board Certification in Family Medicine (for Physician).
  • Current DEA License and CPR Certification (for Physician).
  • Graduate of an accredited Nurse Practitioner or Physician Assistant program (for APC).
  • Credentialed through the American Nurses Association or Physician Assistant Association (for APC).
  • Current California medical license (NP or PA) (for APC).
  • Current Board Certification (for APC).
  • Current DEA License and CPR Certification (for APC).

Nice To Haves

  • Supervise and mentor Advanced Practice Clinicians and students when requested.
  • Participate in after-hours call rotation and community engagement efforts.
  • Consult and collaborate with physician supervisors as needed.
  • Mentor peers and precept students when requested.

Responsibilities

  • Conduct patient histories and comprehensive physical examinations.
  • Diagnose, treat, and manage acute and chronic medical conditions.
  • Order and interpret laboratory and diagnostic tests.
  • Formulate individualized treatment plans based on clinical findings.
  • Educate patients and families on preventive health, lifestyle, and chronic disease management.
  • Collaborate with behavioral health, dental, and pharmacy teams to ensure integrated care.
  • Participate in peer review, quality improvement initiatives, and provider meetings.
  • Supervise and mentor Advanced Practice Clinicians and students when requested.
  • Maintain accurate and timely documentation in the electronic health record (EHR).
  • Participate in after-hours call rotation and community engagement efforts.
  • Consult and collaborate with physician supervisors as needed (for APCs).
  • Develop and coordinate individualized care plans (for APCs).
  • Provide patient education on disease prevention and wellness (for APCs).
  • Participate in quality improvement, required trainings, and peer review (for APCs).
  • Mentor peers and precept students when requested (for APCs).
  • Participate in after-hours call rotation and site committees as needed (for APCs).
