About The Position

The Family Partner ensures families are equal partners in the planning, development and implementation of mental health services at the Sacramento Children’s Home. The Family Partner builds trust with parents and caregivers and assists in navigating systems and supports that agency policies and systems are family friendly, culturally competent and stigma-free. The Family Partner works on integrating advocacy services into the FSPW program. The Family Partner provides peer support to eligible caregivers and guidance in linking to and navigating various systems.

Requirements

Able to read, write, speak and understand the English language.

Ability to utilize an electronic health record to document services.
Able to participate on a management level and represent the agency in multiple venues.
Ability to coordinate meetings, including developing agendas, documenting conclusions, recommendations, actions and follow-up topics.
Intermediate knowledge of Microsoft Word, Excel and Outlook.

Driving is an essential function of this job, and as such, employees in this position will be required to furnish their own vehicle to be used in the course of employment.
A valid CA driver’s license and minimum insurance coverage is also required.
Must be at least 21 years of age.
A High school diploma or GED is required.
CPR/First Aid Certification preferred or obtained within agency-required timeframe.
Maintain regular and reliable attendance, including consistent work schedule adherence, and timely communication regarding absence or schedule changes.
Requires frequent lifting and carrying of materials and equipment, including often handling of items weighing up to 10 pounds and frequently lifting and carrying of items weiging up to 10 pounds.
Requires occasional pushing and pulling of materials, equipment, or supplies weighing up to 10 pounds.
Frequent use of fine motor skills, including grasping, handling, and repetitive hand movements.
Occasionally reaching outward and reaching overhead or over the shoulder.
Frequent repetitive foot movements and walking on normal surfaces or uneven and slippery surfaces on occasional basis while performing essential job duties.
Frequent standing, sitting and occassionally bending and squatting throughout the workday.
Constant visual and auditory awareness is required to be observe particiapants, monitor environments, respond to changing situations, and communicate effectly with clients, families, and staff.
Requires frequent driving to perform essential job duties and travel between work-related locations as needed.
Ability to effectively manage stressful situations and respond to crisis in a timely and appropriate manner.
Able to deal with volatile situations involving clients.
Must be available to work a flexible schedule, which may include evening, night, or weekend hours.
Must be able to perform the essential functions of the position with or without reasonable accommodation.

Nice To Haves

A BA degree in Social Work, Psychology, or a related field is preferred.
Previous experience working in a mental health or social service setting is preferred.
Must have experience as a parent or caregiver of a child with serious emotional disturbances that received service within the public systems including mental health, child welfare or juvenile justice.

Responsibilities

Work directly with assigned caregivers to provide one on one support and peer counseling.
Orient caregivers to the Child and Family Team (CFT) process and High Fidelity Wraparound (HFW) service.
Participate in treatment planning, case conferences, etc. as appropriate.
Provide support, coaching and assistance to caregivers with education rights during special education processes as needed.
Builds trusting, peer-to-peer relationships with parents and caregivers, helping them navigate complex systems, prepare for team meetings, strengthen family bonds, and find sustainable support.
Provides support to families towards stabilization of housing for the purpose of preventing or addressing homelessness.
Collaborate with team, clients/caregivers to develop housing plans.
Develop relationships with landlords.
Partner with clients/caregivers to address housing barriers and provide resources as needed.
Provide intensive case management to support self-sustaining housing.
Identifies and links to resources in the local community which are useful to families within those communities.
Provide individual advocacy, education, and support to meet the needs of families, to include identifying systemic barriers to participating in services.
Provide the parent/caretakers’ perspective and feedback to the clinical staff in order to increase staff awareness of the needs of families and possible barriers to meeting the client’s goals.
Provide support and advocacy to families navigating various systems (i.e., school, housing, CPS, Alta Regional Center, etc.), as appropriate.
Provide information to parents/caregivers to help engage them in forming a positive relationship with treatment team members.
Support parents/caregivers in self advocacy and in making informed decisions regarding the care of their children.
Prioritize participation in client and family meetings as needed.
Document all services to families/clients and services per agency and county requirements.
Utilize existing strengths and strengths to build to advocate for accessibility and inclusion of the child in community activities and resources.
Assist in the ongoing development and evaluation of the service program.
Attend meetings and workgroups as assigned by supervisor to provide a voice for family needs.
Attend trainings/workshops/committees to expand knowledge base, network with other community programs, and provide a voice for families around agency policy.
Attend Adult Peer Support Committee which includes Parent Partners from other provider agencies throughout the county.
Attend and participate in all family child team meetings, team meetings, interdisciplinary team meetings, and other meetings as required.
Establish and maintain collaborative and effective working relationships with internal staff throughout all levels of the agency and with external resources.
Carry out mandated reporter duties as required by law.
Serve as a role model and advocate for children and families involved in the Wraparound process.
Adhere to ethical and professional standards in all aspects of service delivery.
Foster a supportive and empowering environment for children and families to participate in their own journey actively.
Perform other duties as assigned.
