About The Position

Behavioral Health Network (BHN) is seeking a Family Partner to join their Intensive Care Coordination (ICC) team. This role supports parents and caregivers of youth with behavioral needs by utilizing the Family Partner's lived experience to help families navigate systems and build resilience. Requirements

Bilingual - fluent in English and Spanish
Personal experience as a caregiver of a youth with special or behavioral health needs
High School diploma/GED and at least 2 years of experience working with children/adolescents/transition-age youth OR an Associate’s Degree and 1 year of relevant experience OR a Bachelor’s Degree and 1 year of relevant experience
Knowledgeable about navigating child and family-serving systems
Strong communication, organization, and interpersonal skills
Comfortable using electronic systems and documentation tools
Culturally responsive and value diverse lived experiences
Valid driver’s license and access to a reliable vehicle

Responsibilities

Provide peer support to parents and caregivers
Help families identify strengths and develop strategies to meet care plan goals
Educate families on navigating child-serving systems such as DCF, DMH, education, and behavioral health
Empower families by linking them to peer networks and support groups
Connect families with both formal and community-based resources
Foster a culturally responsive and inclusive environment
Maintain accurate and timely documentation
Collaborate with care teams and attend family planning meetings
Actively engage in supervision and ongoing training
