About The Position

Geisinger is seeking a Family Nurse Practitioner (FNP) to join our team supporting care for adults and adolescents based at the Geisinger Behavioral Health Center in Moosic, PA. The FNP will work collaboratively within a multidisciplinary team to deliver high quality, patient centered care. The patient population is 75% adult and 25% pediatric (age 14+), with daily encounters ranging from 10–16. The practice environment includes onsite pharmacy and phlebotomy services, and utilizes the Epic electronic health record. The work schedule is a rotating 3 days on, 4 days off.

Requirements

• Certificate of completion from an approved program for Nurse Practitioners or be eligible to hold national certification that allows the NP to work with the patient populations and location settings associated with this position.
• Must be eligible for licensure as a Nurse Practitioner in the state of PA.
• Master's Degree-Nurse Practitioner (Required)
• Basic Life Support Certification
• Certified Nurse Practitioner - American Academy of Nurse Practitioners
• Licensed Registered Nurse (Pennsylvania)

Nice To Haves

• Minimum 1 year of clinical experience
• Experience using Epic

Responsibilities

• Perform comprehensive medical assessments.
• Provide medication management, evaluations, and daily patient rounding.
• Develop and implement treatment plans in collaboration with psychiatrists, psychologists, nurses, therapists, and case managers.
• Document all care in the Epic electronic health record.
• Participate in treatment team meetings and coordinate care transitions.
• Communicate with families, caregivers, and community providers as needed to support continuity of care.
