About The Position

Intermountain Centers is one of the largest statewide behavioral health and integrated care organizations in Arizona. They are currently recruiting career-minded individuals interested in opportunities within the largest adult and child service continuum in Arizona. The Family Nurse Practitioner provides comprehensive, coordinated primary and preventive health care to patients. This role requires interaction with outside entities, adherence to State of Arizona and contract mandates, and coordination of patient needs with other behavioral health service providers for AHCCCS patients. Documentation in the electronic health record is essential. The role involves providing comprehensive primary care for adults and may include serving as a preceptor for students. Attending monthly provider meetings and staying current with standards of practice are also expected.

Requirements

  • 2 years of experience in certified field.
  • Valid DEA certificate.
  • Current Arizona License to Practice Medicine.
  • Licensed as Family Practice or Internal Medicine.
  • Minimum 21 years of age.
  • Arizona Driver’s License, 39-month Motor Vehicle Report and proof of vehicle registration and liability insurance that meets insurance requirements.
  • Must possess Level 1 fingerprint clearance.
  • CPR and First Aid certification.
  • Initial current negative TB test result.

Responsibilities

  • Serves as the family nurse practitioner to assigned members.
  • Develops differential diagnostic assessments, assessments of capacity for consent to treatment, risk assessments, patient education, and records progress towards treatment goals.
  • Completes medication education and informed consent when possible.
  • Documents all activities in the electronic health record according to policy.
  • Assesses members’ needs and guides the clinical team while managing treatment plan updates.
  • Initiates contact with outside Providers according to establish guidelines.
  • Works closely with other personnel to ensure that each treatment modality is appropriate for the member in addition to working with other healthcare providers involved in the overall care of the recipient of care.
  • Responsible for following any policies, procedures, and controls established by the organization, the HIPAA Privacy Officer, and/or the HIPAA Security Officer regarding access to, protection of, and the use of personal health information (PHI).
  • Performs other duties as assigned or necessary as it relates to the general nature of the position.
  • Provide comprehensive primary care for adults.
  • May serve as preceptor to residents, medical students, and advance practice nursing students.
  • Attend monthly provider meetings.
  • Remain current with standards of practice.
  • Perform other duties or special projects as assigned by medical director.
