The company is looking for a full-time Family Nurse Practitioner for their Stapley Health Center in Mesa and McDowell Health Center in Phoenix. The schedule involves 4-10 hour shifts, with specific days assigned to each location. The role is within an integrated setting and requires establishing therapeutic relationships with patients to achieve treatment goals.

Requirements

  • Nurse Practitioner License
  • Master’s degree in nursing or DNP
  • Valid First Aid/CPR and AED certification
  • Valid DEA license
  • Valid Arizona driver’s license
  • 21 years of age with minimum 3 years driving experience
  • Meet requirements of Terros Health’s driving policy
  • Valid Arizona Fingerprint Clearance card or apply for one within 7 working days of assuming role
  • Pass a TB Test
  • Proficient using computer-based systems including electronic health records (NextGen preferred), Microsoft Outlook, and other Microsoft Office products.

Responsibilities

  • Conducts physicals/check-ups, medical evaluations, and diagnostics.
  • Treats illnesses, orders lab tests, and provides general and preventative care.
  • Prescribes and monitors medications.
  • Coordinates with other providers and makes specialty care referrals.
  • Educates patients about preventative care and wellness activities.
  • Establishes therapeutic relationships with patients for treatment goals.
  • Performs independently or as part of a treatment team.
  • Performs individual treatment and case consultation.
  • Provides leadership and cohesion within the clinic environment.
  • May provide direct supervision to other medical staff and students.
  • Participates in program evaluation procedures and professional record keeping.
  • Makes referrals appropriately for specialty and behavioral health care.
  • Provides direct patient services not less than 85% of the work time.
  • Employs therapeutic interventions appropriate for identified treatment issues.
  • Seeks appropriate clinical supervision/support when necessary.
