About The Position
Copa Health is seeking a compassionate, mission-driven, full-time Family Nurse Practitioner to join its integrated primary care team serving the West Valley Phoenix metropolitan area. The role delivers high-quality, person-centered care in an integrated behavioral health setting. The successful candidate is dedicated to improving health outcomes for a diverse member population and works collaboratively in a multidisciplinary environment.
Requirements
- Hold a current and valid Family Nurse Practitioner license in the State of Arizona.
- Hold national certification as a Family Nurse Practitioner.
- Have a DEA license or be able to obtain one upon hire.
- Have strong communication skills and a collaborative approach to care.
- Complete pre-employment criminal background and drug testing.
Nice To Haves
- Experience in outpatient or primary care settings.
- Behavioral health experience.
Responsibilities
- Provide comprehensive primary care, including assessment, diagnosis, and treatment of members across the lifespan.
- Manage chronic and acute conditions in collaboration with behavioral health providers, nursing staff, and case managers.
- Complete evaluations, medication management, and follow-up care in accordance with best practice standards.
- Maintain accurate and timely documentation in the electronic health record.
- Participate in integrated care team meetings, quality initiatives, and performance improvement projects.
- Promote whole-person wellness through education, prevention, and culturally responsive care.
