About The Position
Come join our team as a full-time Family Nurse Practitioner at Community Health Services- 2221 Hayes Avenue in Fremont. This is an outstanding opportunity for an experienced, compassionate FNP, who enjoys caring for a diverse population of patients--including both adults and children. Applicants must have at least 2 years of experience in a family practice setting. As a Federally Qualified Health Center, our providers are eligible to apply for student loan repayment through National Health Service Corps (NHSC)- up to $75,000 for full-time employees.
Requirements
- Current License to practice in the State of Ohio as a Family Nurse Practitioner
- Current Basic Life Support Certification
- Minimum of two years as a FNP preferred
- Knowledge of documenting with EHR
- Strong communication skills and the ability to multi-task
- Ability to critically think and problem solve
- Reliable, professional, and compassionate
Responsibilities
- Work directly with patients Monday through Friday- No Weekends
- Perform Adult and Pediatric Assessments
- Communicate clearly with Providers, Patients, and CHS Staff
- Follow-up on Lab and Diagnostic testing
- Follow all safety, HIPAA, and regulatory guidelines
