About The Position

Midland Care Connection is hiring a Family Nurse Practitioner on behalf of APRN Physician Services and APRN Hospice & Palliative Care. This role involves providing comprehensive, patient-centered primary care to older adults and individuals residing in rehabilitation centers, skilled nursing facilities, assisted living communities, and independent living settings. The Nurse Practitioner will assess and manage acute and chronic health conditions, monitor patient health changes, develop individualized treatment plans, and support patients through recovery and long-term care. A key aspect of the role is collaborating with physicians, nurses, caregivers, families, hospice, home health, rehabilitation, and other healthcare professionals to ensure coordinated and continuous care. Responsibilities include performing physical assessments, ordering and interpreting diagnostic tests, managing medications, addressing acute illnesses and chronic diseases, providing dementia and geriatric care, and educating patients and families on treatment options and goals of care. The position also involves providing advanced clinical care to patients with serious or life-limiting illnesses, focusing on comfort, symptom management, and quality of life within Midland Care Connection’s Hospice and Palliative Care program. This includes evaluating patients, diagnosing and treating medical conditions, managing complex symptoms, prescribing medications, and developing or updating individualized care plans. The Nurse Practitioner will work closely with patients and families to understand goals of care, explain treatment options, and provide guidance throughout the hospice or palliative care journey, while also collaborating with physicians and an interdisciplinary team to ensure care aligns with patient wishes. By providing medical care directly within residential and long-term care settings, the Nurse Practitioner aims to improve accessibility, continuity of care, comfort, and overall quality of life, while reducing unnecessary hospital visits and supporting patients through health changes.

Requirements

  • Current, unrestricted Nurse Practitioner/APRN license in the state of Kansas
  • Current national board certification as a Nurse Practitioner through an approved certifying body
  • Graduate degree from an accredited Nurse Practitioner program
  • Current DEA registration and ability to prescribe medications
  • Current BLS certification
  • Experience providing direct patient care as a Nurse Practitioner
  • Strong assessment, clinical decision-making, and communication skills
  • Ability to work collaboratively as part of an interdisciplinary care team
  • Ability to provide compassionate, patient-centered care to individuals with serious or life-limiting illnesses
  • Ability to work independently and manage multiple patient needs while maintaining accurate clinical documentation

Nice To Haves

  • Experience in hospice, palliative care, geriatrics, oncology, or another setting caring for patients with serious or life-limiting illnesses
  • Hospice and Palliative Medicine (HPM) certification or willingness to obtain it
  • Experience with symptom management, pain management, and end-of-life care
  • Experience discussing goals of care and advance care planning with patients and families
  • Experience working within an interdisciplinary team
  • Familiarity with Medicare hospice regulations and hospice eligibility requirements
  • Experience providing care in patients’ homes, long-term care facilities, or other community-based settings
