About The Position

Under the direct clinical supervision of the Medical Director or Clinical Services Director, the Family Nurse Practitioner (FNP) delivers top-of-license, culturally responsive primary and preventative healthcare across the entire lifespan. Serving as a primary healthcare provider within a multidisciplinary team, the FNP conducts comprehensive health assessments, manages acute and chronic conditions, orders diagnostic testing, and prescribes appropriate pharmacologic and non-pharmacologic therapies. Operating within the Delivery (The Gold Standard) framework, this role emphasizes high-touch patient coaching, preventative disease management, and active collaboration with specialized navigators and behavioral health professionals to improve health outcomes in the community.

  • Demonstrated ability to establish effective, respectful working relationships across multidisciplinary clinical teams, non-clinical staff, and external partners.
  • Clear, empathetic, and effective verbal and written communication skills; strong clinical problem-solving under pressure.
  • Excellent organizational skills, attention to detail, and ability to manage clinical flow efficiently.
  • Minimum of 3 years of clinical experience in a Family Nurse Practitioner role (community health, FQHC, or safety-net clinic experience preferred).
  • Graduate of an accredited School of Nursing and an accredited Nurse Practitioner Program (Master's level or higher).
  • Active, unencumbered California Advanced Practice Registered Nurse (APRN) and Nurse Practitioner (NP) license.
  • Active California Furnishing License and DEA Registration.
  • Board Certification (AANP or ANCC) in Family Practice (or board-eligible with expected certification within 12 months of hire).
  • Current BLS and ACLS Provider Cards (PALS provider card preferred).
  • Proficiency in Electronic Health Records (EHR) and basic administrative software (Microsoft Office Suite / Google Workspace).

Responsibilities

  • Provide ongoing primary care, preventive screenings, urgent/acute care visits, annual physicals, and chronic disease management across pediatric, adolescent, adult, and geriatric populations.
  • Order, perform, and interpret diagnostic, laboratory, and therapeutic tests relative to presenting clinical issues and age-specific guidelines.
  • Prescribe appropriate pharmacologic agents and non-pharmacologic interventions within the California APRN scope of practice and organizational guidelines.
  • Monitor care plan progress and clinical outcomes for complex, high-risk patients, adjusting care plans as clinically indicated to support optimal patient health.
  • Partner closely with Primary, Specialized, and Intensive Navigators, behavioral health specialists, and community health workers to deliver holistic, team-based care management.
  • Monitor and follow up on specialty care, radiology, laboratory, and community resource referrals to ensure seamless care continuity.
  • Actively participate in integrated clinical care management conferences, interdisciplinary team reviews, and high-risk case huddles.
  • Provide culturally grounded health education, disease self-management coaching, and preventative guidance to patients and families.
  • Serve as a trusted patient advocate, helping community members navigate complex health systems and overcome barriers to care.
  • Timely, accurately, and thoroughly document all patient encounters, assessments, treatments, and care plans within the Electronic Health Record (EHR) system.
  • Adhere to clinical protocols, quality assurance standards, and state regulatory requirements.
  • Attend mandatory clinical, organizational, and quality improvement meetings as required.
