About The Position

We are seeking a compassionate and patient-centered advanced practice clinician to support primary care and community-based outreach services in the Princeville and Mendota area. In this role, you will provide high-quality, evidence-based care to patients across the lifespan, helping improve access to health services and promoting better outcomes in the communities we serve. Community Health Partnership of Illinois is a 501 c 3 not-for-profit that provides primary preventative health care to uninsured and underserved community residence throughout northern and central Illinois, providing access to medical, dental, behavioral health and linkage to other social service. Our focus is delivering quality, culturally responsive health care services to our patients, caregivers, and partners in the communities we serve.

Requirements

Master's or doctoral degree in nursing.
Licensed as a Registered Nurse.
Certification as a Family Nurse Practitioner.
Prior experience working in primary care or a related field.
Maintain licensure, CPR/BLS and other certifications through continuing education and professional development activities.

Bilingual/Spanish is preferred but not required.

Responsibilities

Providing primary care to patients of all ages, from infants to elderly individuals.
Conducting physical examinations.
Ordering and interpreting diagnostic tests.
Diagnosing and treating common illnesses and injuries.
Prescribing medications.
Managing chronic conditions.
Providing preventive care and health education.
Referring patients to specialists when necessary.
Collaborating with other healthcare providers to ensure comprehensive and coordinated care for their patients.
Conducting research.
Participating in quality improvement initiatives.
Contributing to the development of healthcare policies and practices.
