Family Nurse Practitioner
About The Position
St. John’s Community Health (SJCH) is seeking a Family Nurse Practitioner (NP) to provide comprehensive, whole-person care to patients. This role involves advancing the mission of health equity by delivering high-quality, community-based primary care in a collaborative, interdisciplinary environment. The NP will work alongside medical, dental, and behavioral health professionals to deliver integrated services. SJCH is a Federally Qualified Health Center (FQHC) with over 60 years of experience delivering quality, accessible care to underserved communities in Southern California. They are driven by a commitment to equity, compassion, and community impact.
Requirements
- Master’s or Doctoral degree from an accredited Nurse Practitioner program
- Completion of an accredited Family Nurse Practitioner program
- National board certification (AANP or ANCC)
- Active, unrestricted California NP license
- NP Furnishing Number (or eligibility)
- Active DEA registration
- BLS certification
- Familiarity with Electronic Health Records (EHR) systems
- Strong commitment to health equity, cultural humility, and caring for diverse communities
Responsibilities
- Provide comprehensive primary care for patients across the lifespan
- Conduct physical exams; order, interpret, and manage diagnostic testing
- Diagnose and manage acute and chronic medical conditions
- Collaborate with multidisciplinary teams to deliver integrated, whole-person care
- Educate patients and families on disease prevention, treatment plans, and healthy lifestyle choices
- Document patient care accurately and timely in the EHR system
- Support SJCH’s mission through clinical service, outreach, and community engagement
