POSITION OVERVIEW: Title: Nurse Practitioner Reports To: Medical Director Schedule: M-F; 8am-5pm Status: Exempt Summary: The Nurse Practitioner is a licensed independent practitioner who is responsible for meeting, assessing, and diagnosing patients in our clinic and providing them with healthcare services that meet State and National standards for caregiving. The provider assumes responsibility and accountability for health promotion and maintenance including the use and prescription of pharmacologic and non-pharmacologic interventions. They will be responsible for prescribing patients with appropriate medication, health regimens and lifestyle adjustments that will contribute to their recovery from diagnosed health issues, prevent future health issues and promote a healthier life.

Requirements

Bachelor of Science in Nursing (BSN) and a Master's or Doctorate in Nursing from an accredited program.
Active and unrestricted Nurse Practitioner (NP) license in the state of California.
DEA registration.
Prescriptive authority in California.

Experience working with Native American population in a healthcare setting 2+ years preferred.

Responsibilities

Independently performs comprehensive history and physical examinations for perioperative and primary care patients, assessing both normal and abnormal findings.
Evaluates and treats acute injuries and illnesses, provides preventive healthcare services, and monitors the effectiveness of interventions.
Orders and interprets diagnostic and therapeutic tests, prescribes medications, and implements treatment plans in compliance with standardized procedures, federal and state regulations, and Indian Health Service (IHS) guidelines.
Coordinates follow-up services, including lab work, imaging, specialist referrals, and scheduling of appointments through the Medi-Cal Receptionist.
Ensures seamless transitions between healthcare settings, managing admissions, transfers, and discharges while collaborating with a multidisciplinary team.
Consults with the Medi-Cal Director or Provider Practice Lead (PPL) when cases exceed their professional scope and assists in updating clinic protocols as needed.
Provides patient and family education on health promotion, risk management, and disease prevention.
Interfaces with community organizations to identify and connect individuals with necessary resources.
Contributes to staff development by acting as a resource in their area of expertise.
Engages in continuing education, upholds licensure requirements, and participates in quality assurance initiatives, ensuring that patient records adhere to QA guidelines.
Updates Medi-Cal forms and clinic protocols, assists with inventory management, and participates in the Quality Assurance Committee.
Notifies and coordinates with the Department of Health Services, local health departments, and fire authorities in the event of public health concerns such as epidemic outbreaks, poisonings, major accidents, or other critical incidents.
Reports public health concerns within 24 hours via telephone, followed by written confirmation, with an incident report retained on file for one year.
Plays a crucial role in delivering high-quality, patient-centered care while ensuring compliance with all regulatory standards and fostering a healthier community.
