About The Position

This Family Nurse Practitioner will provide full spectrum primary care and play a leadership role to build a high-quality, integrated, and patient-centered clinical practice. This position works collaboratively with and reports to the Chief Medical Officer (CMO), as well as: 1) works closely with other interdisciplinary team members including LCSW, Medical Assistant (MA), and Licensed Vocational Nurse (LVN); 2.) provides support and oversight over the MAs and LVN ., and 3.) oversees CLIA-waived laboratory operations, including training, quality oversight, and regulatory readiness, to ensure safe, efficient, and high-quality care delivery. This role will uphold and champion the agency’s mission, vision, and values and contribute to a collaborative and inclusive work culture.

Requirements

  • Licensed Family Nurse Practitioner in the State of California.
  • Relevant years of experience: Minimum of 3 years’ clinical experience/care experience in a federal qualified health center or community health center working, with preference for prior work with an underserved Spanish-speaking population.
  • DEA registration preferred at time of hire and required within 60 days of employment.
  • Two or more years of case management and triage experience working in a primary care clinic setting.
  • Must hold or be eligible to obtain a current BLS (Basic Life Support) Certificate. (The selected candidate will have a 30-day grace period to obtain your certificate from the date of hire.)
  • Must be eligible for credentialing and privileging in accordance with CHC policies and procedures.
  • Must be eligible for enrollment and participation as a provider for Medi-Cal, Medicare and contracted health plans.
  • Knowledge of CLIA, NCQA, Title 22, and applicable local, state, and federal regulations.
  • Spanish Bilingual Required, both spoken and written.
  • Knowledge and experience with Electronic Health Record programs.
  • Demonstrates and models excellent “customer service” orientation and ability to work with diverse populations.
  • Exercises extreme discretion with the ability to hold, balance, and transmit public and confidential information needed by various parties.
  • Proficient in MS Office and Google Workspace products.
  • Valid CA Driver’s license required.

Responsibilities

  • Provides comprehensive full spectrum primary care for patients of all ages, including pediatric, women’s, adult, geriatric populations and minor procedures at La Familia Community Health Center.
  • Listens to and understands internal and external patient/client needs
  • Delivers evidenced based care that supports our aim for improved health outcomes, exceptional patient experience and efficient use of resources.
  • Performs therapeutic procedures, such as injections, immunization, suturing and wound care, and managing infections.
  • Offers culturally and linguistically appropriate care, particularly to the large Latino and Spanish-speaking families.
  • Serve as a member of the collaborative cross-functional team, including behavioral health clinicians, care coordinators, and medical assistants to ensure whole person, team-based care.
  • Serve as a member of the cross-functional team of clinicians and other members consulting on assessment, management and treatment of complicated cases, chart review, co-signing, and case conferences.
  • Consult with physicians on the management, assessment and treatment of specific cases as needed, and per protocol.
  • Participate in clinic start-up activities, contributing to workflow design, process and quality improvement, development of care protocols and integrated care integration
  • Document accurately and efficiently in the electronic health record and comply with clinical, regulatory and organizational quality standards, including timeliness.
  • Oversee the LVN/MA as they provide assessment and provide care to patients.
  • Support CMO with the evaluation of clinic and laboratory workflow to assure efficiency and quality health care.
  • Assists in the overall coordination of the registered nurse activities, including assignments to various areas in the clinic to ensure that coverage of all areas is provided for.
  • Suggests improvements for, patient scheduling, and service delivery, and collaborates professionally with interdisciplinary team members, including other clinicians, medical assistants, and case managers.
  • In conjunction with Clinic Director/Administrator support laboratory operations to ensure efficiency and timeliness in provision of services by constant enforcement of agency standards.
  • Recommends policies and procedures, cross-site training, competency testing for point of service testing personnel.
  • Supports point of service CLIA-waived testing as needed by answering technical questions, giving in-services on new equipment and procedures, proficiency testing and providing new product information.
  • Provides guidance and subject matter expertise regarding available tests.
  • Provide day-to-day supervision, guidance, and support to assigned staff.
  • Set clear expectations, delegate tasks effectively, and monitor staff performance.

  • Lead and colead regular team meetings, check-ins, and coaching sessions.

  • Support to temporarily fill in unstaffed roles until the position is filled.

  • Communicate staffing needs and operational impacts to leadership promptly.
  • Complete assigned work on or before deadlines in accordance with directives, policies, standards, and prescribed procedures.
  • Ensure staff complies with established work schedules, attendance, and punctuality requirements.
  • Monitor and approve employee timecards, including daily clock-in/out, breaks, overtime, and leave.
  • Ensure employees take required meal and rest breaks in compliance with labor laws and agency policy.
  • Submit approvals on time to support accurate payroll processing.
  • Investigate and resolve timesheet discrepancies and escalate unresolved issues to HR/Payroll.
  • Enforce organizational policies, procedures, and compliance with applicable labor laws.
  • Monitor, understand, and effect key onsite clinic metrics that relate to quality outcomes.
  • Support Quality Assurance processes.
  • Supports the Clinic Director/Administrator in preparation and management of CLIA site inspections as they occur
  • Supports the Clinic Director/Administrator investigate all physician, patient, and departmental complaints by direct contact with involved parties and documentation of follow-up actions taken.
  • Participates in Medical Operations team meetings to provide technical laboratory information and advice as needed.
