About The Position

Nexus Neurorecovery Center – Conroe is a specialized neurorehabilitation program dedicated to serving adults with complex neurological, behavioral, and medical conditions. Situated on a beautiful 26-acre wooded campus north of Houston, our facility provides highly specialized inpatient rehabilitation and long-term care services for individuals recovering from traumatic brain injuries, stroke, spinal cord injuries, and other neurological conditions. Our programs include the Rise Program, focused on intensive neurological rehabilitation, and the Life Care Program, designed to support individuals requiring long-term residential care. Requirements

  • Master’s Degree in Nursing as a Nurse Practitioner OR Master’s Degree in Nursing with completion of a post-master’s Nurse Practitioner Certification Program required.
  • Current Texas Registered Nurse (RN) license required.
  • Current Texas Advanced Practice Registered Nurse (APRN-NP) license required.
  • Current BLS certification required and must remain active throughout employment.
  • Behavioral management and de-escalation training required per facility guidelines.

Nice To Haves

  • Three (3) to five (5) years of experience preferred in long-term rehabilitation, neurorehabilitation, or neurobehavioral healthcare settings.
  • Experience working with neurologically impaired or behaviorally complex patient populations strongly preferred.
  • Certification in behavior management or related methodologies (SAMA, ABA, PIP, etc.) preferred.

Responsibilities

  • Performs a variety of advanced clinical duties involved in assessing, treating, controlling, and preventing illness and disease within the facility.
  • Provides general medical care and treatment to patients/residents under the direction of the Medical Director and in collaboration with the interdisciplinary treatment team.
  • Participates in the observation, assessment, diagnosis, intervention, evaluation, rehabilitation, patient education, and ongoing medical management of adults with complex neurological, behavioral, and rehabilitation needs.
  • Ensures services provided are consistent with the Texas Nursing Practice Act and all applicable regulatory standards.
  • Obtains detailed patient histories and performs comprehensive physical examinations.
  • Assesses and treats acute and chronic medical conditions while providing direct patient care.
  • Performs emergency and life-saving procedures as necessary.
  • Communicates with supervising physicians regarding unusual findings, emergencies, or conditions requiring physician consultation.
  • Practices proper safety, environmental, infection control, and behavioral management procedures.
  • Documents patient progress, treatment interventions, and clinical findings in the medical record accurately and timely.
  • Performs complete physical and neurological examinations utilizing interviews, health histories, physical assessments, and diagnostic testing.
  • Orders and interprets laboratory and diagnostic studies and intervenes appropriately based on findings.
  • Prescribes and administers medications and treatments in accordance with Texas APRN regulations and facility guidelines.
  • Recommends and coordinates institutional and community resources to support patient and family needs.
  • Collaborates with physicians, therapists, nursing staff, and interdisciplinary team members to manage acute and long-term medical needs.
  • Ensures continuity of care between physician visits through ongoing patient monitoring and follow-up.
  • Participates in rotating 24-hour medical call coverage.
  • Provides education and clinical support to staff members.
  • Participates in admissions meetings and weekly interdisciplinary treatment team meetings.
  • Assists in developing individualized healthcare plans and goals for physician review.
  • Recommends rehabilitative interventions including physical therapy, occupational therapy, speech therapy, and behavioral supports as appropriate.
  • Performs other duties as assigned.
