Family Nurse Practitioner/ Physician Assistant - GHC
About The Position
Valley-Wide Health Systems, Inc. (Valley-Wide) is seeking a Family Nurse Practitioner/Physician Assistant to provide medical care at its Guadalupe Health Center in Antonito, CO. This is a full-time position within a non-profit, Federally Qualified Health Center (FQHC) that serves a large, multi-county area in southern Colorado. The ideal candidate will be positive-minded, passionate about serving rural communities, and committed to delivering personalized, integrated healthcare across the lifespan. Valley-Wide is an approved site for the Colorado Health Service Corps and National Health Service Corps loan repayment programs, offering eligible providers up to $90,000.00.
Requirements
- Current unencumbered State of Colorado Medical Practitioner Licensure
- Current unrestricted DEA License
- Successfully complete a background check and drug testing
Nice To Haves
- Bilingual skills (English/Spanish) desirable but not required
Responsibilities
- Perform physical examinations, record and document physical findings within the EMR System (NextGen).
- Review data and health history to formulate a definitive diagnosis based on patient’s condition.
- Order, interpret, and evaluate diagnostic tests to identify and assess patient’s clinical problems and healthcare needs.
- Prescribe pharmacological and non-pharmacological therapies.
- Review and evaluate treatments to determine success.
- Collaborate with the integrated healthcare team to determine a comprehensive patient healthcare plan.
- Refer patients to other healthcare practitioners for consultations or specialized health resources for treatment as appropriate.
- Educate patients and their families on treatment or healthcare needs.
- Assist in the oversight of support personnel with clinical guidance.
- Perform other duties assigned by employer from time to time that are commensurate with professional services, normally and customarily, performed by a physician.
