Primary Care Birth+ Family Nurse Practitioner or Physician Assistant

This practice serves a diverse patient population, including many Spanish-speaking families, making it an excellent opportunity for a provider who enjoys working with multicultural communities. Build meaningful, long-term relationships with patients while making a lasting impact on the health of the community. The role is open to Family Nurse Practitioners (FNP) and Physician Assistants (PA) who will care for patients from birth through adulthood. This is a 100% outpatient primary care - Family Medicine, Birth+ position.
Requirements
- Unrestricted State of Connecticut Medical License (or eligible)
- FNP (Family Nurse Practitioner) or Physician Assistant training
- 1+ years’ experience in a primary care or similar setting
Responsibilities
- Perform routine physical examinations.
- Formulate medical, educational, and psychosocial plans to treat patients.
- Develop plans for further evaluation of health needs in consultation with physician(s) and care team members, e.g., referral to other health team members or specialists.
- Provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols.
- Identify problems outside of their scope of practice and refer appropriately to physician.
