The Family Navigator is responsible for the Family Map, which will drive all services for the family. This role involves working collaboratively with children and families, utilizing the NCFAS, any additional assessments, and family input to create a plan of care for the family. The position also entails creating a resource atlas complete with many local resources to assist the family advocate and families in getting resources needed to strengthen the families and create supports to achieve long term outcomes. The Family Navigator ensures family advocates adhere to trauma informed care values and fidelity to the Link model, and engages in regular networking activities to keep the resource atlas up to date.
