About The Position

Evergreen Life Services is seeking compassionate, reliable individuals and families across Tennessee to become Family-Model Providers. In this role, you will open your home and heart to provide care for an individual with an intellectual, developmental, or physical disability, or an elderly individual. By welcoming them into your home as a family member, you will help them develop daily life skills, encourage personal choice, and support them in achieving their goals.

Requirements

  • Must be at least 21 years old (25 or older if working with youth/adolescents under DCS programs).
  • Must live in a stable Tennessee residence (owned or rented) that meets all state and HUD safety guidelines.
  • Must have a private, available bedroom that meets placement regulations.
  • Must be financially stable and able to meet your own existing household needs independently.
  • Ability to pass all state-mandated criminal background checks, fingerprinting, and a successful home study completed by Evergreen.
  • Must possess a valid driver's license, clean driving record, reliable vehicle, and active auto liability insurance.
  • Complete all mandatory Evergreen pre-service training, including CPR/First Aid certification.

Responsibilities

  • Provide compassionate assistance with daily tasks, including personal hygiene, eating, and meal preparation.
  • Safeguard the individual’s physical health through accurate medication administration and transporting/accompanying them to medical appointments.
  • Help individuals master daily life skills and foster their independent living capabilities.
  • Assist individuals participating in the Employment and Community First (ECF) CHOICES program with community integrated supports, transportation, and daily routines that foster consistent employment.
  • Offer a warm, inclusive home environment. Provide ongoing companionship and involve the individual in household and community social activities.
  • Keep detailed, daily records of health metrics, behavior tracking, and goal milestones as required by Evergreen and state regulations.
  • Must comply with all terms and conditions specified in the independent contractor agreement.
