About The Position

AGL is seeking compassionate individuals to become Family Model Providers. In this role, you will open your home and heart to support adults with disabilities, helping them achieve personal goals and greater independence. This is an independent contractor position offering a flexible schedule and competitive compensation. You will provide daily living support, encourage skill-building, facilitate community integration, and ensure the safety and rights of the individuals you serve. Comprehensive training and ongoing staff support are provided.

Requirements

• Must be able to provide a safe, welcoming home environment for the individual(s) you support.
• Strong communication, empathy, problem-solving, and organizational skills.
• Must pass all required background checks and comply with Medicaid and Tennessee Department of Disability and Aging regulations.
• Reliable transportation for medical appointments, errands, and community activities.

Nice To Haves

• Previous caregiving or direct support experience is preferred but not required; AGL offers training and ongoing support.

Responsibilities

• Assist with personal care, meal planning/preparation, medication reminders, and household tasks.
• Encourage independence by helping individuals develop and practice life skills (e.g., budgeting, cooking, using technology).
• Plan and participate in community outings and social activities, fostering genuine connections and inclusion.
• Ensure the individual’s safety, respect their rights, and advocate for their interests within the home and community.
• Communicate regularly with AGL staff and participate in team meetings to align on care plans and goals.
