About The Position

Advanced Primary Care (APC) - Eugene, part of Praxis Health, is seeking an experienced Family Medicine Physician to join our growing Eugene practice. APC is reimagining what primary care can look like through an innovative, value-based model focused on access, prevention, population health, and meaningful patient relationships. Rather than simply treating illness as it arises, our providers proactively manage chronic conditions, close care gaps, and partner with patients to help them stay healthy and out of the hospital. This is an exciting opportunity for a physician who enjoys both the practice and evolution of primary care. Requirements

MD or DO with Board Certification/Board Eligibility in Family Medicine
Active or eligible for Oregon medical licensure
Minimum of 2 years of post-training primary care experience required
Strong foundation in comprehensive adult primary care and chronic disease management
Experience with routine outpatient procedures
Interest in preventive medicine, population health, and value-based care
Comfortable practicing in a growing environment where processes continue to evolve
Collaborative, flexible, and willing to contribute beyond traditional role boundaries when the team needs support
