About The Position

Altais Medical Group is seeking a Family Medicine Physician to join their growing team-based primary care model. This role provides clinical primary care, focusing on comprehensive first contact and continuing care for patients. The physician will manage direct patient care, implement evidence-based medicine, and adhere to practice guidelines. Responsibilities include patient advocacy, coordinating health care services for cost-effectiveness, promoting patient-provider communication, and driving clinical initiatives. The position involves health promotion, disease prevention, health maintenance, counseling, patient education, Annual Health Assessments, and Chronic Care Management. The physician will collaborate with interdisciplinary teams, leverage the Electronic Health Record (EHR) for patient care management, and ensure clinical compliance.

Highly organized, strong time management skills and flexibility to manage multiple timelines and activities.
Board certification must be current.
The PCP is required to provide call coverage for assigned Brown & Toland members 24/7, 365 days a year or participate in a call group.
Minimum of 1-2 years clinical experience
Successfully completed residency in their specialty
Active and in good standing Medical Licensure in the state of California and Board Certified
Active and good standing NPI number
Active and good standing DEA number from US Drug Enforcement

Nice To Haves

Preferred: 3+ years professional experience working as a PCP
Significant experience working with an EHR, preferably Epic
