Responsibilities

Provide outpatient primary care, including diagnosis, treatment, and preventive services
Manage chronic and acute medical conditions
Collaborate with a multidisciplinary care team to optimize patient outcomes
Maintain thorough and accurate electronic medical records
Participate in quality improvement initiatives and care coordination
