About The Position
Fort HealthCare, located in beautiful Fort Atkinson, Wisconsin, is seeking a Board Certified/Board Eligible Family Medicine Physician to join their dedicated team in providing primary care. The Family Medicine Clinic offers a collaborative, integrated care environment, which includes shared space with Cardiology, Pulmonology, and Endocrinology Services, fostering seamless coordination and comprehensive care for our patients. Requirements
- Board Certified/Board Eligible Family Medicine Physician
- BC/BE
Responsibilities
- Provide primary care as a Family Medicine Physician.
- Work in a collaborative, integrated care environment.
- Coordinate care with in-house specialties including Cardiology, Pulmonology, and Endocrinology.
- See an average of 18-20 outpatient visits per day.
- Participate in four clinic days per week (32 patient contact hours) with one administrative day.
