About The Position

Physician Affiliate Group of New York (PAGNY) has an opportunity for a per diem Family Medicine Physician - Health Equity for Adolescents in the Legal System (HEAL) with the Administration for Children's Services (ACS). PAGNY staffs several municipal hospitals and clinics within New York City through an affiliation agreement with NYC Health + Hospitals . PAGNY’s corporate staff provides administrative, practice management, and human resources services in support of its more than 4,000 clinical providers. The HEAL Program is a new NYC Health + Hospitals initiative to transition into being the primary provider of medical services to young people in City custody. The staff will be responsible for 24/7 medical care across two secure detention sites, two school-based health centers, and limited pharmacy services at seven non-secure sites (group homes). Services include comprehensive medical screenings, dental screenings and procedures, vaccinations and routine testing, responses to immediate need, and referrals for offsite care within and outside the NYC Health + Hospitals system. Requirements

  • Current New York State Physician registration.
  • Board Certified in Family Medicine.
  • Minimum three years related experience in family, pediatric, adolescent urban populations.
  • Must pass all appropriate screenings to work in a secured setting with young people.

  • Spanish language proficiency preferred (Not Required).

Responsibilities

  • Provide medical care to ACS adolescents keeping safety and security measures paramount.
  • Take relevant patient history, i dentify patient needs, and make diagnoses.
  • Develop and implement treatment plans using diagnostic data.
  • Perform complete well-care examinations, including physical examinations.
  • Perform complete gynecological histories and physical examinations.
  • Write prescriptions and inform patients about medications prescribed; garner parent/guardian consents when needed.
  • Complete medical forms and documentation related to patient care.
  • Make and track all referrals to other agencies and providers, with follow-up care provided.
  • Assist with phlebotomy, as needed, to ensure smooth patient flow.
  • Administer immunizations, as needed.
  • Distribute, refill, discard, and discontinue medication for patients.
  • Complete Court Orders and all other ACS mandated paperwork.
  • Evaluate and treat ACS employees (ONLY on an emergent basis).
  • Provide health education on topics including, but not limited to, preventive medicine and counseling on chronic diseases, as well as parenting, prenatal, nutrition, family-planning, medication usage, and stress management.
