About The Position

This is an exceptional opportunity for a Family Practice Physician to join a busy, well-respected practice, supported by a strong referral base and the full resources of a robust health system network. Join a collaborative, multidisciplinary team with 3 Physicians and 2 APP’s along with RN and support staff. Access to onsite lab and Xray services. Call structure with other sites: 1:17. Practice setting: Outpatient clinic; average of 15 patients per day, serving patients of all ages with flexibility to increase volume based on provider preference and community need. Employed position within a nonprofit health system. Deliver comprehensive patient care across outpatient and inpatient settings, including evaluation, diagnosis, treatment, care coordination, and consultation, with opportunities to manage referrals and hospital-based responsibilities as needed. Engage in quality improvement, cost-effective care initiatives, and academic activities as desired, while maintaining timely, accurate documentation and collaborating with a multidisciplinary team. Care for a loyal and growing patient base in Little Chute, WI with a population of 12,000 and surrounding population in the Fox Valley of 80,000. Located in the heart of the Fox Cities, Little Chute offers small-town charm with access to a regional population providing a strong patient base and community feel. Conveniently situated near Appleton, the area features excellent schools, outdoor recreation along the Fox River, and an easy drive to larger metro areas like Green Bay and Milwaukee. Requirements

  • Physician MD/DO credentialed from the Minnesota Board of Medical Practice obtained prior to hire date or job transfer date.
  • Physician MD/DO credentialed from the Wisconsin Medical Examining Board obtained prior to hire date or job transfer date.
  • Doctor of Medicine (MD) or Doctor of Osteopathy (DO) required.

Responsibilities

  • Deliver comprehensive patient care across outpatient and inpatient settings, including evaluation, diagnosis, treatment, care coordination, and consultation.
  • Manage referrals and hospital-based responsibilities as needed.
  • Engage in quality improvement, cost-effective care initiatives, and academic activities as desired.
  • Maintain timely, accurate documentation and collaborating with a multidisciplinary team.
