About The Position

We’re hiring a Family Medicine Physician in Michigan City, Indiana who wants a mission-driven outpatient role with leadership opportunities, structured workflows, and long-term stability. You’ll step into an outpatient primary care role focused on proactive primary care, chronic disease management, preventive services, and whole-person care. You’ll work alongside an established multidisciplinary team with integrated behavioral health and nutrition services, allowing you to provide comprehensive, patient-centered care. You’ll practice in Michigan City, Indiana, a vibrant Lake Michigan community offering beautiful beaches, outdoor recreation, a revitalized downtown, and convenient access to both Chicago and South Bend.

Requirements

• Board Certified or Board Eligible in Family Medicine (MD or DO)
• Completed an accredited Family Medicine residency
• Hold or can obtain an active Indiana medical license
• Passionate about serving underserved communities
• Value quality improvement and structured care processes
• Thrive in a collaborative, team-based environment

Responsibilities

• Outpatient primary care focused on proactive primary care, chronic disease management, preventive services, and whole-person care.
• Work alongside an established multidisciplinary team with integrated behavioral health and nutrition services.
• Provide comprehensive, patient-centered care.
• Leadership and clinical guideline development opportunities.
• Teaching opportunities with residents and students.
