About The Position

Ascension is seeking an experienced Family Medicine Nurse Practitioner (NP) to join their team for a new primary care facility launch in Waukesha, WI. This role involves leading patient assessments, diagnostic evaluations, and preventative screenings, while collaborating with physicians on complex health conditions. The NP will also counsel patients and families on wellness, manage referrals, coordinate community resources, and maintain electronic health records. The facility serves a growing suburban population and is part of a national nonprofit Catholic health system.

Requirements

• Master's degree of Nursing required.
• Nurse Practitioner credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date.
• Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date.
• One or more of the following: Nurse Practitioner credentialed from the Minnesota Board of Nursing obtained prior to hire date or job transfer date.
• One or more of the following: Registered Nurse credentialed from the Minnesota Board of Nursing obtained prior to hire date or job transfer date.
• Nurse Practitioner work experience in a Family Med or walk-in clinic setting is required.

Nice To Haves

• BLS Provider. American Heart Association or American Red Cross accepted.
• Drug Enforcement Admin credentialed from the Drug Enforcement Administration.

Responsibilities

• Lead patient assessments, diagnostic evaluations, and preventative screenings.
• Collaborate with physicians to diagnose and treat complex acute and chronic health conditions.
• Counsel patients and families on preventive wellness and treatment options.
• Manage direct specialty referrals and community resource coordination.
• Maintain rigorous electronic health record documentation.
