About The Position
Adelante Healthcare is seeking a Family Medicine Nurse Practitioner or Physician Assistant to provide high-quality, comprehensive primary care to vulnerable populations across 9 clinics in Maricopa County. The role involves preventive care, health promotion, diagnosis and management of acute and chronic conditions, patient education, and community-focused care. The position offers autonomy within a collaborative care team and a mission-driven environment focused on serving others.
Requirements
- Graduate of an accredited Nurse Practitioner program.
- Active Arizona NP & RN or PA license.
- Current DEA certification.
- CPR/AED certification (AHA or Red Cross).
- Strong clinical judgment & patient communication skills.
- Commitment to community health and underserved populations.
- Valid AZ Driver’s License.
Nice To Haves
- 2+ years in community health.
- Bilingual (English/Spanish).
Responsibilities
- Provide high-quality, comprehensive primary care.
- Deliver preventive care & health promotion.
- Diagnose, treat & manage acute and chronic conditions.
- Educate patients and provide community-focused care.
- Coordinate care and make referrals as needed.
